Carers’ Q & A - August

This quarter we are having a session focusing on Advance Care Planning.

What is Advance Care Planning?

Have you thought about what would happen if you were not around to make decisions for the person you care for? Or what would happen if you were unable to make decisions for yourself?

Advance care planning is making a plan for the type of health or personal care you would want to receive if you lost the ability to make decisions.

This session will be an introduction to Advance Care Planning (ACP).

Topics covered will include:

  • Basic understanding of what is Advance Care Planning

  • The benefits of Advance Care Planning

  • What Advance Care Planning documents are available, including:

    • Medical Treatment Decision Maker

    • Advance Care Directive

  • How to complete the forms

  • Understanding the legislation surrounding Advance Care Planning

There will also be time for any questions you may have.

Carer’s Question & Answer sessions will be held quarterly.
